Impact of an inducement to give high doses of amikacin and gentamicin on serum concentrations in critically ill patients with severe sepsis

چکیده
Methods Single-center observational study. All ICU patients with clinical indication of aminoglycosides were eligible.ICU physicians were encouraged to administer maximal recommended doses of amikacin and gentamicin (30 and 8 mg/kg, respectively). The first and subsequent doses and corresponding peak plasma concentrations were recorded. Guideline targets for serum concentrations were used with ≥60 and ≥30 mg/L for amikacin and gentamicin, respectively. A target pharmacokinetic/ pharmacodynamic (PK/PD) ratio of 10-times the minimal inhibitory concentration (10xMIC) was also measured.

Impact of 30 mg/kg amikacin and 8 mg/kg gentamicin on serum concentrations in critically ill patients with severe sepsis.
OBJECTIVES Low first-dose peak serum concentrations of amikacin and gentamicin are commonly reported in ICU patients. The present study aimed to assess whether 30 mg/kg amikacin or 8 mg/kg gentamicin achieved target concentrations in ICU patients with severe sepsis. PATIENTS AND METHODS Sixty-three ICU patients (Simplified Acute Physiology Score II = 43 ± 16) with severe sepsis and an indicat...
متن کاملTherapeutic monitoring of amikacin and gentamicin in critically and noncritically ill patients
OBJECTIVE Therapeutic drug monitoring (TDM) enables individualization in the treatment to optimize clinical benefit and minimize drugs' side effects. Critically ill septic patients represent a challenge for antimicrobial treatment because of pathophysiological impact of sepsis on pharmacokinetics of drugs. The aim of this study was to assess the appropriateness of gentamicin and amikacin dosing...
متن کاملAtorvastatin for Prevention of Amikacin-induced Electrolytes Imbalances; a Randomized Clinical Trial
Aminoglycosides are still widely used for treatment of gram-negative sepsis in critically ill patients. The most reported electrolyte abnormalities related to these drugs are hypokalemia, hypomagnesemia, and hypocalcemia. In this study potential benefit of atorvastatin in prevention of amikacin-induced electrolytes imbalances has been evaluated. In this trial 44 patients were assigned to the at...

متن کاملHigh-dose amikacin for achieving serum target levels in critically ill elderly patients
Introduction To achieve target concentrations, the application of higher-than-standard doses of amikacin is proposed for the treatment of sepsis due to an increase in volume of distribution and clearance, but little data are available on aminoglycoside administration in critically ill elderly patients. Patients and methods Forty critically ill elderly patients (aged over 65 years) who require...
